If you choose to move forward with your service, you will complete a telehealth visit with a mid-level or higher provider through Master Monarch Injectors to obtain medical clearance prior to your treatment. Texas state regulations require this clearance to be in compliance. This will be renewed annually. * Cost of the telehealth medical clearance visit is included in this charge. No additional charge for this visit will be added unless a treatment is received.
